    OBJECTIVE Describe the implementation and effects of Mobile Acute Care for Elders (MACE) consultation at a Veterans Affairs Medical Center (VAMC). DESIGN Retrospective cohort analysis. INTERVENTION Veterans aged 65 or older who were admitted to the medicine service between October 1, 2012, and September 30, 2014, were screened for geriatric syndromes via review of medical records within 48 hours of admission. If the screen was positive, the MACE team offered the admitting team a same-day consultation involving comprehensive geriatric assessment and ongoing collaboration with the admitting team and supportive services to implement patient-centric recommendations for geriatric syndromes. RESULTS Veterans seen by MACE (n = 421) were compared with those with positive screens but without consultation (n = 372). The two groups did not significantly differ in age, comorbidity, sex, or race. All outcomes (30-day readmission, 30-day mortality, readmission costs) were in the expected direction for patients receiving MACE but did not reach statistical significance. Patients receiving MACE had lower odds of 30-day readmission (11.9% vs 14.8%; odds ratio [OR] = 0.82; 95% confidence interval [CI] = 0.54-1.25; p = .360) and 30-day mortality (5.5% vs 8.6%; OR = 0.64; CI = 0.36-1.12; p = .115), and they had lower 30-day readmission costs (MACE 15,502;CI=15,502; CI = 12,242-19,631;comparison=19,631; comparison = 18,335; CI = 14,64114,641-22,962; p = .316) than those who did not receive MACE after adjusting for age and Charlson Comorbidity Index. CONCLUSION Our MACE consultation model for older veterans with geriatric syndromes leverages the limited supply of clinicians with expertise in geriatrics. Although not statistically significant in this study of 793 subjects, MACE patients had lower odds of 30-day readmission and mortality, and lower readmission costs. J Am Geriatr Soc 67:818–824, 2019

    Analysis of current density and related parameters in spinal cord stimulation

    A volume conductor model of the spinal cord and surrounding anatomical structures is used to calculate current (and current density) charge per pulse, and maximum charge density per pulse at the contact surface of the electrode in the dorsal epidural space, in the dorsal columns of the spinal cord and in the dorsal roots. The effects of various contact configurations (mono-, bi-, and tripole), contact area and spacing, pulsewidth and distance between contacts and spinal cord on these electrical parameters were investigated under conditions similar to those in clinical spinal cord stimulation. At the threshold stimulus of a large dorsal column fiber, current density and charge density per pulse at the contact surface were found to be highest (1.9·105 ¿A/cm2 and 39.1 ¿C/cm2 ·p, respectively) when the contact surface was only 0.7 mm 2. When stimulating with a pulse of 500 ¿s, highest charge per pulse (0.92 ¿C/p), and the largest charge density per pulse in the dorsal columns (1.59 ¿C/cm2·p) occurred. It is concluded that of all stimulation parameters that can be selected freely, only pulsewidth affects the charge and charge density per pulse in the nervous tissue, whereas both pulsewidth and contact area strongly affect these parameters in the nonnervous tissue neighboring the electrode contact

    In contrast to the widespread assumption that dorsal column fibers are the primary targets of spinal cord stimulation by a dorsal epidural electrode, it appears that dorsal root fibers are recruited as well, and even preferentially under various conditions. This will, however, limit the coverage of the painful body areas with paresthesia, a prerequisite for the management of chronic pain. In order to favor the preferential stimulation of dorsal column fibers, advantage was taken of the different positions and orientations of fibers in the dorsal columns and dorsal roots.     Stability of the global ocean circulation: The connection of equilibria within a hierarchy of models

    We address the problem of the multiple equilibria of the thermohaline circulation in a hierarchy of models. The understanding of the relation between bifurcation diagrams of box models, two-dimensional models and those of a global ocean general circulation model, is facilitated through analysis of the equilibrium solutions of a three-dimensional Atlantic-like sector model with an open southern channel. Using this configuration, the subtle effects of the wind-stress field, the effects of continental asymmetry and the asymmetry in the surface freshwater flux can be systematically studied. The results clarify why there is an asymmetric Atlantic circulation under a near equatorially-symmetric buoyancy forcing. They also lead to an explanation of the hysteresis regime that is found in models of the global ocean circulation. Both explanations are crucial elements to understanding the role of the ocean in past and future climate changes
